• We have plenty of parking all along the street on 5th Avenue or a few spaces behind our building including a disabled parking spot
• Please do not park in the building parking lots adjacent to our building as you may be towed. 

• If you are having a life threatening emergency call 911.Dr. Mo is on call for all of his patients 24/7 every day personally and can be reached for emergencies by calling  817-423-8585, which goes directly to our answering service and then to Dr. Mo.
• Non emergency calls made to the answering service - the message will be taken and faxed to the office for a response the following business day

It will depend entirely upon the purpose of your referral and appointment – 

we will let you know if you need testing prior and if so will obtain prior authorization to permit this

WHAT HOSPITAL IS DR MO 

ASSOCIATED WITH?

Dr. Mo has chosen to practice exclusively at Baylor Scott and White All Saints and Baylor Heart and Vascular Hospital in Forth Worth.

Dr. Mo rounds daily as needed and coordinates your care through his personally selected hospitalist team

You wil see Dr. Mo if needed in the hospital and/or  following your stay as quickly as possible in office followup

• Your medications are always called in with refills unless Dr. Mo or his associates specify otherwise
• In general, please call your pharmacy for your refills
• If any issues or concerns arise, you or your pharmacy can call us and we are always happy to help
• If you are on your last refill please call us 2 weeks prior and we will contact your pharmacy 
• We are happy to send 90 day prescriptions to prescription services, if your medications are deemed appropriate for such

• All major commercial insurances and Medicare plans will be accepted. 
• We are in the process of contracting with many of theses commercial payors so please be understanding as we complete this transition to the new practice
• We also accept self-pay

DOES DR. MO RECOMMEND THAT HIS PATIENTS GET  VACCINATED AGAINST COVID-19?  WHERE CAN I GO?

• Yes, if you have no contraindications.  Dr. Mo is vaccinated.
• We do not get to pick which vaccine we get, we get the one that is available at the time we become eligible.
• Taking the vaccine and maintaining CDC precautions is the only way we can together end this pandemic
• Please visit your county health department website to receive instructions on where to receive the vaccine AND go the CDC.gov for more questions to inform your decision

• Cardiac testing is sophisticated and complex to interpret and share
• When Dr. Mo has finalized interpretation for in house testing or has received results from outside sites, and reviewed, you will be able to see these in your secure access patient portal; link located on this site
• Discussion of your testing results ALWAYS takes place in person at your next appointment or if more urgently, by special appointment
• We will call you immediately with any critically abnormal findings, and give you instructions on what we want you to do

• As long as necessary to make sure your care needs are met
• We do our very best to be as on time as possible, but cardiac medicine, especially given the logistical and safety protocol mandated challenges posed by the pandemic, is complex
• When Dr. Mo is running behind from the hospital, running behind in office, or if he is called to an emergency we will update you to give you every opportunity to decide if you wish to keep your appointment time or reschedule

WILL I ALWAYS SEE DR. MO

• All new consultations and acute followup visits will be seen by Dr. Mo
• As our practice has grown, Dr. Mo has identified and recruited the most qualified physician extenders to participate in your care
• Amy Blancke is our first NP, is a critical cardiac care nurse and NP who has make great contributions to our practice
• Our nurse practitioners see routine followup visits, video visits, or cardiac clearances for surgery
• Their work is always supervised and signed off by Dr. Mo; he will make every effort to come in to your visit with our NPs

• From the secure patient portal – you can fill everything out online or print out forms
• The more you complete prior to the visit electronically, the more efficient we can be to afford you more time with Dr. Mo or his associates

• Your records are always yours, just call our office at 817-423-8585 or send an email to staff@drmoheart.com to request and we will take care of this for you.
• Records are sent free of cost to any other practice or hospital for continuity of care
• A personal copy for your records may incur a small fee

• Surgical clearance is a serious matter that require serious attention
• We do not believe in completing templated forms – we will see you, examine you, and integrate all of your cardiac data so that we can provide the requesting surgeon and anesthesia team our best opinion
• Please give this time and plan well ahead of time (AT LEAST 2-4 weeks) for non urgent medically necessary surgery - we cannot accelerate this for you for standard requests
